
Expanding on our revised 5e fear effects, today we are looking at monsters in the monster manual that can inflict the frightened condition. Or to use our new terminology, 3 levels of fear. Being limited to the monster manual, we can literally just type out our list here in alphabetical order. When reading these, its important to note that a saving throw against a fear effect is automatically granted at the end of a turn when the originator is not visible, or is no longer a combatant.
General Rule: After you pass, immune to that effect for 24 hours. This becomes “A creature can become immune to this effect if they pass a save and have 0 levels of fear, or have reduced their fear level to 0 after failing the save initially.” This means if a PC fails a save, and takes 4 stacks of fear, but then saves a few times, to get to 2 stacks. The monster can use the ability again to bring them back to 4 (maximum) stacks.
- Androsphinx: Instead of the first two roars giving the frightened condition. The first gives a stack of fear, no save. The second gives two stacks, with a save for half.
- Banshee: Horrifying Visage: Grants a level of fear each round.
- Beholder: Functions as written. This is designed to emulate the cause fear spell.
- Beholder Zombie, Death Tyrant, Spectator: See above.
- Chain Devil: Unnerving Mask: this ability only grants two stacks of fear, but does not end at the end of the creature’s turn.
- Cloaker: Moan: There is no saving throw for the initial effect, but it only grants a single level of fear. The saving throw can be done as normal.
- Demilich: Howl: this ability grants 4 stacks of fear, not 3 and does not end on its next turn.
- Dragon (all): Frightful Presence: This gives 4 stacks on a failed save and 2 on a passed.
- Ghost: Horrifying Visage: Grants a level of fear each round.
- Lich: Frightening Gaze: This ability grants 3 stacks of fear.
- Mummy: Dreadful Glare: Grants 3 stacks of fear, 4 on a failed save.

- Nalfeshnee: Horror Nimbus: This ability grants 3 stacks of fear.
- Pit Fiend: Fear Aura: This ability gives 4 stacks of fear.
- Quasit: Scare: This ability gives 3 stacks of fear.
- Revenant: Vengeful Glare: this ability gives 2 stacks of fear, 4 on a failed saving throw.
- Satyr: Satyr Pipes: Frightening Strain: All creatures within 60ft gain a level of fear, save negates.
- Scarecrow: Terrifying Glare: 4 stacks of fear, save for 2 stacks.
- Sea Hag: Horrific Appearance: This is a single stack of fear, but a creature is never immune to it.
- Tarrasque: Frightful Presence: This gives 4 stacks on a failed save and 2 on a passed.
